Oak Hills offers physical therapy, occupational therapy and speech therapy. We also provide specialized therapy services such as diathermy, E-stim, ultrasound and a dynamic standing/balance machine. Programs, and care plans for your family member or loved one.

Dr. Berkowitz has special interests in Wound Care, Reconstructive Foot Surgery, Sports Medicine and Diabetic Foot Care. Dr. Berkowitz graduated from the Ohio College of Podiatric Medicine in 2004 where he served as an adjunct clinical faculty member. He completed a three year surgical foot and ankle residency at University Hospitals Health System/Richmond Heights where he served as a clinical attending. During his post-graduate residency, Dr. Berkowitz received specialized training in Pediatric Lower Extremity Surgery in Vienna, Austria. Dr. Berkowitz is double American Board Certified in Podiatric Medicine and Wound Healing. A facility van provides transportation to doctor’s appointments, outing and other needs as they arise.

In her personal time, Gwen enjoys riding motorcycles and belongs to a non-profit organization which organizes motorcycle benefits to raise money for local families in need. She considers herself blessed to be able to use her favorite hobby to help others.

Whispering Pines is our Memory Care unit, which offers personalized treatments for those dealing with dementia or Alzheimer’s. This secure memory care facility works to enable and empower residents in a comfortable, safe environment, allowing them to focus on their strengths and be an active participant in their daily care. Gathering around the table for meals is important in our community. Our highly experienced chef creates three delicious meals daily and offers menu-style options. Fried catfish served on Fridays is a resident favorite.

Our skilled nursing and rehabilitation services are readily available to our long-term care patients. Long-term care also includes infection control and contracted palliative and hospice care. From home health to skilled nursing and now in assisted living/memory care. She has a broad understanding of senior care and continues to grow and learn. Gwen joined the Frontier Management family in 2019 as the Business Office Manager for Oak Hills Terrace. Since then, she has transitioned to the Assistant Executive Director and now Executive Director.
Our nutritionist and chef provide three delicious meals per day, and snacks are available anytime. We even have a beauty salon and barbershop to keep our residents looking and feeling their best. Oak Hill includes a separate, secure area specifically designed to meet the needs of those with memory loss.
